Headaches and neuropathic pain

Pain is one of the leading causes of disability in the world’s population. A law (No. 38 of 3/15/10) requires Italy to duly recognize and treat pain in all its expressions and in all health care settings.

The goals

The focus of the research area is the functional reeducation of pain when it loses its finalistic role by permanently debouching beyond its physiological protective function. Special attention is given to chronic headache pain, a condition that afflicts 4 percent of individuals in our nation, particularly affecting the age groups of highest work and social productivity and whose direct costs alone amount to about 2,900 euros per patient per year.

Area interests

Translational research in the research area aims to integrate genetic and molecular-biological knowledge with clinical and instrumental paradigms in order to restore this not insignificant portion of the population to social and occupational validity. It consists of the following fronts:

Epidemiological
Through the establishment of the national chronic headache registry pursues aims:
– clinical care (identification of affected individuals, ascertainment of drug-resistance, assurance of care);
– health-economic (regulating patient access to modern treatment-rehabilitation procedures while exercising clinical governance over the various centers located throughout the country);
– medico-legal (civil disability);
– scientific (creation of the world’s first national data base, phenotype-biotype correlation study using biobanking and system-medicine approach).

Neurophysiological
– use of laser sensory evoked potentials to delineate and monitor alterations in nociceptive systems during primary or secondary chronic pain (vascular, traumatic, inflammatory, degenerative neurological diseases) and to assess the endogenous pain control network (DNIC: diffuse noxious inhibitory control) in order to better target functional recovery programs;
– Use of noninvasive electrical stimulation systems of the vagus nerve and supra orbital nerve in chronic cephalic pain.

Genetic
– Identification of molecular and genetic markers related to the evolution toward chronicity of neuropathic and neurovascular pain, looking for a phenotype-biotype correlation that would allow personalizing the rehabilitation program and predicting the possibility of recovery

Pharmacological
– Implementation of therapeutic protocols with drugs in clinical trials

 

All areas of research
