MODE OF ADMISSION
Contact the intake office:
phone 0864 2507427 and 0864 2507424
fax 06 52255638
Monday through Friday 8: 00 – 13:00 / 15:00 – 18:00.
Saturday: 8:00 am – 1:00 pm. It is possible to download the admission form directly from our website.
The form should be completed by the home hospital, i.e., the attending physician; applications without the signature, stamp and telephone number of the physician will not be accepted.
USERS FROM OTHER HOSPITALS
For such users, it is necessary to contact reception in advance, which will provide all the necessary information for the correct submission of the application for admission. Applications for hospitalization are evaluated within 24/48 hours of request. The outcome (acceptance, request for further clarification, non-acceptance), will then be communicated by fax to the requesting hospital, and if the application for admission is accepted, the patient will be placed on the waiting list; the date of admission will be communicated as soon as the bed becomes available.
USERS FROM HOME
Within 48 hours of submitting an application for hospitalization, the user or family member can, after a phone call to the Acceptance Office, learn the outcome of the application. If successful, the user is placed on the waiting list.
The date of admission will be communicated by phone.
The reason for any negative outcome can only be communicated in person to the user, the user’s guardian or a person delegated by the user.
NEEDED FOR HOSPITALIZATION
Whether coming from another health care facility or directly from home, upon admission the User should bring with him/her:
WAITING LISTS.
SSN services are booked and managed through shared agendas with the ASL No. 1 Cup Avezzano-Sulmona-L ‘Aquila (https://cup.asl1abruzzo.it), according to the priorities indicated in the commitment by the proposing physician.
Inpatient services are booked directly through the facility’s CUP via e-mail (cup.sulmona@sanraffaele.it) or telephone contact (0864/2507400), and waiting lists, which are almost nonexistent in this case, are realized anyway based on the availability of the specialist physician, operators and health technicians.
